Secure Form is a service of Fieldstone Health, LLC. One set of terms covers this website and fieldstone-health.com, and the agreements a healthcare organization signs with us take precedence over them where they differ.
Using this website
The Fieldstone Health Website Terms of Use apply to anyone using secure-form.com or fieldstone-health.com. They cover acceptable use, what you send us, our content, disclaimers and the limits on our liability. By using this website you agree to them.
Free demos
Your free first form, and any other form we build before your service is active, is a demo for fictional patient information and blank templates only. The person who starts a demo accepts the Free Demo Terms for their organization before using it.
Customers
Using any form with real patients requires a signed Services Agreement, a Business Associate Agreement with the service schedule for your plan (Direct Delivery or Workflow Portal), and our activation of the service. Those agreements govern fees, term, liability and how patient information is handled, and they prevail over the website terms. Fees are on the Pricing page and in each Order Form, never in the agreements themselves.
