Both plans deliver every completed form securely. The difference is what happens next: a PDF in an inbox, or one visible workflow for the whole team.

How long we keep completed forms

Direct Delivery
We email each completed PDF to your office and keep the submission for 30 days, so we can send it again if an email goes astray. Then we delete it. Backup copies expire within a further 30 days, so every copy is gone within about 60 days. Direct Delivery submissions are never archived.
Workflow Portal
Submissions stay in your portal for as long as you're a client, unless you ask us to set a shorter period. Your staff can't delete them; we remove junk or duplicates when you ask. If your practice is covered by HIPAA, each submission is also kept for one year in a locked backup archive that protects it against accidental deletion, changes and tampering. Your staff work from the portal; the archive is our safety copy.
If you cancel
With Direct Delivery, any submission we still hold is returned securely if you ask, up to 30 days after the end. With Workflow Portal, you get one free export if you ask, up to 10 days after the end. Then we delete your submissions, forms and account data within 30 days, and backups expire within a further 30. Locked archive copies can't be deleted early, even by us, so each one is deleted when its one-year lock ends.

Pages are counted from the form you send us. Send us your forms and we'll confirm a fixed price before any work begins. Forms that need integrations with other systems, calculated fields or major cleanup get a custom quote.

Offer your form in Spanish.

$25 per page · $75 minimum

Let patients switch between English and Spanish within the same form. Provide your approved Spanish wording, or we can arrange professional medical translation at an additional cost, confirmed before work begins.

Pricing covers adding the Spanish version to your form. Professional translation is separate.

After launch

Small changes are included. Larger ones are quoted first.

Your annual service plan includes small updates to keep your forms useful as your needs change.
